A claim that it is covered so it was paid, while the coverage trail is missing, is not this paid. A coverage opinion, an FNOL acknowledgment, a reserve set, or a verbal "it was paid" while required coverage evidence is missing is payment theater, and it is not this covered. A payment claim alone is not proof the named coverage evidence was on the file. A verbal "it was paid" alone is neither. Coverage evidence alone is not payment of that covered successor outcome. A firm can hold a disbursement story and still not have the loss inside the granted coverage grant. A firm can hold that coverage grant and still lack cash or indemnity disbursed on an accepted claim.

A coverage grant with no payment evidence behind it is not this paid. Payment has to trail back to the coverage evidence, and the coverage evidence has to show policy language matching the loss. A payment package that floats free of that trail is not this paid. What changes Tuesday is the refusal to let one record wear the other record name.
